Admin area:Manage Surveys (account menu)

Click Account menu - Surveys to enter the page

Account Survey page looks as follows

It has list of surveys and form to add new Surveys.

After you added new survey you can edit it.

You can apply survey to new Live Chat button on Add Live Chat Button -> Advanced page.
